This closing survey is for customers who have completed the DC MOMS Program. Please DO NOT complete this closing survey if you have not completed DC MOMS. The DC MOMS program offers an 8-week stress management course to work-eligible mothers receiving cash assistance through the Temporary Assistance for Needy Families (TANF) Program. The course teaches practical skills for managing life stress and improving mood, including relaxation breathing, problem solving, and communication skills. If you are interested in joining DC MOMS or learning more about the program, please send an email to [email protected].

This assessment includes a series of questions that will help the Department of Human Services (D.H.S.) learn more about you, your family, and the goals you have so we can support you on your journey. From this assessment, we hope to provide an opportunity for you to think about your future and what would be helpful for moving in that direction. Some of the information you provide will help D.H.S. connect you to the most appropriate coach to guide you towards reaching your goals, along with additional resources that you may need. If you do not want to complete this assessment on your own, then you may schedule an appointment with a D.H.S. staff member in the Office of Work Opportunity (O.W.O.). Pregnant Women & New Mother Resources
The Pregnant Women and New Mothers Resource Guide provides customers with information on additional resources they can access because they are pregnant or have a new child. Examples include additional food assistance to purchase healthy foods for expectant mothers and formula, breastfeeding support and lactation services, and a free pack n play for safe sleep. These resources are helpful as you prepare for a new baby or adjust to life with a new baby.
